By booking an appointment or receiving services at MADE Med Spa, LLC (“MADE”), you agree to the following terms and conditions. Please read them carefully. If you do not agree, please contact us before proceeding with any treatment.

The services provided at MADE are medical aesthetic treatments performed by or under the supervision of licensed medical professionals, including Karlyne Bassam, RN, and overseen by Dr. Linda Myers, MD. All treatments carry inherent medical risks, which will be disclosed to you during your consultation and informed consent process.

Prior to any treatment, you will be required to complete a thorough intake form and sign an informed consent document. This document outlines the risks, benefits, alternatives, and aftercare requirements specific to your treatment. No service will be rendered without signed consent.

Aesthetic outcomes vary by individual. MADE does not guarantee specific results. While we strive to deliver exceptional, personalized care, factors including skin type, lifestyle, adherence to aftercare, and individual biology influence treatment outcomes. Results from previous clients shown in before/after images are not a guarantee of identical outcomes.

All treatments are performed based on clinical assessment. We reserve the right to decline, modify, or reschedule services if, in our professional judgment, it is not safe or appropriate to proceed.

Payment is due at the time of service. We accept major credit cards and applicable payment methods as displayed at checkout. Completed treatments are non-refundable. If you are dissatisfied with your experience, please contact us directly so we can address your concerns.

Membership fees, packages, and prepaid services are non-transferable and non-refundable unless otherwise specified in writing at the time of purchase.

You are responsible for providing complete and accurate health history, including all medications, supplements, allergies, and medical conditions, prior to receiving treatment. MADE assumes no liability for adverse outcomes resulting from undisclosed health information.

Clients under the age of 18 must have written parental or guardian consent prior to receiving any service. Certain treatments are not available to minors regardless of consent.

MADE Med Spa, LLC and its staff shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services beyond the amount paid for the specific treatment in question. Nothing in these terms limits liability for gross negligence or intentional misconduct.

These terms are governed by the laws of the Commonwealth of Virginia. Any disputes arising from your use of MADE's services shall be resolved in the courts of Fairfax County, Virginia.
